Home Units


Our Home Phototherapy Consultants will require some basic information, along with a prescription from your doctor. Most insurance companies also require that we furnish a Letter of Medical Necessity from your doctor and supporting medical records.

You can send us your basic information by filling out this form or calling us at (216) 831-0600. Our Smart Form makes it easier for your dermatologist to generate your prescription and Letter of Medical Necessity. Feel free to share it with your provider, then direct them to e-mail it to [email protected] or fax it to (216) 765-0271. If you already have a prescription, you are welcome to upload it below.

Why can’t I purchase a unit immediately online?

We cannot sell units online through a traditional ecommerce platform because a home phototherapy unit is an FDA-cleared device that requires a prescription.

What happens once you receive my materials?

Our Insurance Support Team will review your materials to:

  • Ensure that we have all information required to execute a sale
  • Check your documents for errors or inconsistencies that could cause your insurer to deny a claim for a home phototherapy unit, and
  • Assess whether your insurer traditionally requires pre-authorization for the purchase of a home phototherapy unit.

A Home Phototherapy Consultant will then reach out to tell you how much your unit will cost and discuss next steps. We do not post prices online because sales terms vary by insurance contract.  

If you intend to purchase your unit without insurance or if your insurance plan does not require pre-authorization, we could close your sale that same day, provided that the prescription from your doctor has no errors and we are not missing any information.

If your file requires extra work, we have staff who are dedicated to following up with insurance companies and doctors’ offices on behalf of patients so that you don’t have to, saving you time. We spend hours sitting on hold with insurance companies to advocate for our customers every day.

Do you offer financing?

Yes, we offer installment loans with no down payment in partnership with CareCredit. Payment plans last 6 to 60 months. 0% APR financing is available on payment plans lasting 6 to 24 months.* You can apply for financing online or over the phone with the assistance of one of our phototherapy consultants.

Do I need to schedule an installation appointment?

Only if you purchase a Foldalite. Our other units ship fully assembled or are easy to set up. Our team will provide you with instructions to help you set up your unit in your home. If you think you will require assistance, we can help you contract an installer who lives in your area through a partner service organization. We generally recommend that elderly patients purchasing larger units contract an installer.

Do I need to purchase any additional accessories?

Yes, you need to own a set of phototherapy goggles. Most units come with goggles, but we can sell you extras.

Please note that National Biological Corporation is not responsible for coverage decisions made by your insurance provider. If your insurer denies coverage for your unit for any reason, you are responsible for paying for it.
