Instructions

Our Home Phototherapy team will require some basic information, along with a prescription from your doctor. Many insurance companies also require a Letter of Medical Necessity from your dermatologist and supporting medical records.

You can send us your basic information by filling out this form or calling us at (216) 831-0600. Our Smart Form makes it easier for your dermatologist to generate your prescription and Letter of Medical Necessity. Feel free to share it with your provider, then direct them to e-mail it to [email protected] or fax it to (216) 765-0271. If you already have a prescription, you are welcome to upload it below.

Why can’t I purchase a unit immediately online?

Home phototherapy products are FDA-cleared medical devices requiring a prescription. For this reason, we cannot sell units online through a traditional e-commerce platform. Be wary of phototherapy products being sold online without a prescription.

What happens after I submit my information for a phototherapy device?

Our Medical Benefits team will process your submitted materials to make this easy on you:

  • We will ensure we have all required information to quickly supply you with a home phototherapy system
  • Contact your insurance company as needed to get you the coverage you deserve, and
  • We will work with your insurer and dermatologist for pre-authorizations where required

A friendly Phototherapy sales associate will contact you to discuss your insurance coverage and any costs you may incur as you begin home phototherapy. Insurance plans vary widely, ranging from full coverage to no coverage.  

If you intend to purchase your unit without insurance coverage or if your insurance plan does not require pre-authorization, we can potentially ship your home phototherapy system right away.

Whatever the case, our team is dedicated to following up with insurance companies and doctors’ offices on your behalf so that you don’t have to, saving you time, and getting you the treatment you need. If necessary, we will dedicate hours on hold with insurance companies to advocate for our customer’s skin care.

Do you offer financing?

Yes, we offer installment loans with no down payment in partnership with CareCredit. Payment plans last 6 to 60 months. 0% APR financing is available on payment plans lasting 6 to 24 months.* You can apply for financing online or over the phone with the assistance of one of our Phototherapy sales associates.

Do I need to schedule an installation appointment?

Our Phototherapy systems either ship fully assembled or are easy to set up. Our team will provide you with instructions to help you get setup in your home. If you think you will require assistance, we can help you contract an installer who lives in your area.

Do I need to purchase any additional accessories?

Your Phototherapy system will come with everything you need to get started; however, extra eye protection goggles are available for purchase as needed.
